What to Expect During Your First Chiropractic Visit

Walking into a chiropractor's office for the first time can feel a little uncertain, especially if you have spent years hearing different opinions about what chiropractic care actually involves. The truth is that a first visit at The Spine Works looks much more like a thorough medical workup than the quick crack-and-go experience some patients picture. Dr. Zack Farmer's process is built around understanding your story, examining your spine and nervous system carefully, and then delivering a clear plan you can decide on with confidence.
Before You Arrive
Most patients book online through our Jane portal, which lets you choose a time that works for you and complete your intake paperwork from home. Filling that out beforehand saves you time in the office and lets Dr. Farmer review your health history before he even meets you. Plan to arrive a few minutes early on the day of your visit. Wear comfortable clothing you can move in. Our office is at 301 14th Ave N STE 101 in Nashville, with parking right out front.
The Consultation
Your appointment begins with a sit-down conversation. Dr. Farmer wants to know what brought you in, when the problem started, what makes it better or worse, and what you have already tried. This is not a rushed five-minute interview. The history is one of the most important parts of the visit because the way symptoms behave usually tells us more about the underlying cause than any single test result. Be prepared to describe past injuries, accidents, surgeries, sports backgrounds, work demands, and sleep habits. All of it matters.
The Examination
After the consultation, Dr. Farmer performs a physical, neurological, and orthopedic examination. He will look at how you stand, walk, and move through specific ranges of motion. He may test reflexes, muscle strength, sensation, and the response of certain joints to motion. He will palpate the spine and surrounding muscles to feel for areas of restriction, tenderness, or asymmetry. Posture is evaluated from the side and the back, and specific tests will help identify whether the pain is coming from a disc, a nerve, a joint, or soft tissue. None of this is painful, and Dr. Farmer will explain what he is checking and why as he goes.
Digital X-Rays Onsite
If imaging is appropriate, we have on-site digital X-ray equipment in our office, which means we can capture the exact views Dr. Farmer needs without sending you somewhere else. Digital X-rays use low-dose technology and produce immediate, high-quality images. They show the alignment and condition of your spine, allow Dr. Farmer to rule out red flags like fractures or significant degeneration, and help him design a treatment plan that fits your unique anatomy. Not every patient needs imaging on day one, but when they do, it is convenient and fast.
Your Report of Findings
Once the exam and X-rays are complete, Dr. Farmer reviews everything with you in plain language. You will see your own X-rays, hear what he found in the exam, and understand exactly what is going on with your spine. This is also when he explains what kind of care he recommends, how often, for how long, and what outcomes are realistic. He will also be honest if your situation is better suited for a different specialist. The goal of the report is for you to leave the appointment knowing exactly what is happening with your body and what your options are.
Your First Treatment
Many patients receive their first treatment on the same day as their evaluation, though this depends on findings. A typical first visit may include a corrective chiropractic adjustment tailored to your specific spinal segments, gentle muscle work, and any modalities that are appropriate, which can include spinal decompression therapy or SoftWave therapy depending on the diagnosis. Adjustments are generally quick and comfortable. Some produce a popping sound, which is just gas releasing from a joint, similar to opening a soda. Many patients feel immediate improvement in mobility, and some feel a little soreness for the first day or two as the body adjusts.
What Happens After
You will leave with a clear plan: how often you should come in, what home care or stretches to do, and what milestones to expect at the two-week, four-week, and beyond marks. Dr. Farmer reassesses regularly and adjusts your plan based on how you are responding. The goal is to get you out of pain, restore proper function, and then build a foundation that keeps the problem from coming back.
How Long Will It Take?
Plan on about an hour for your first visit. Subsequent appointments are much shorter, typically fifteen to twenty minutes. We respect your time and we run on schedule.
